Mechanic Banjo Bantolo Bolingot attests that he installed the sidecar on Maulion's motorcycle. 3. They executed this joint affidavit to submit to the Land Transportation Office for registration of the motorcycle with a sidecar.

JOINT AFFIDAVIT OF OWNER AND MECHANIC


(For Registration of Motorcycle with Sidecar)

WE, MARLON MARANAN MAULION, of legal, married, Filipino and a


resident of Brgy. Pachoca, Calapan City, Province of Oriental Mindoro and BANJO
BANTOLO BOLINGOT, of legal, Filipino and a resident of Brgy. Palhi, Calapan City,
Province of Oriental Mindoro, after having duly sworn to in accordance with law, hereby
depose and state that:

1. I, MARLON MARANAN MAULION, am the present OWNER of a


motorcycle that I purchased on February 08, 2021 from Edmund Almanon
Gamier, of legal age, Filipino and a resident of Brgy. Aurora, Municipality of
Naujan, Province of Oriental Mindoro, more particularly described as follows:

Make : Honda
Series : CCG125WHJ
Body Type : Motorcycle
Engine No. : KSW10E365614
Chassis No. : KSW10363266
Year Model : 2019

2. I am planning to use the said motorcycle as a private service vehicle with


a sidecar;

3. I, BANJO BANTOLO BOLINGOT, am a MECHANIC, and by virtue of this


JOINT AFFIDAVIT, I hereby attest that I was the Mechanic who caused the
installation of the sidecar to the above-described motorcycle;

4. We are executing this JOINT AFFIDAVIT to serve as an additional


requirement to be submitted to the Land Transportation Office for the
registration of the said motorcycle so that the same can be registered as one with
a sidecar (“motorcycle with sidecar”);

5. We are executing this JOINT AFFIDAVIT to attest the truth of the foregoing
facts and for the purpose as stated above, and for any other legal intents it may
serve.

FURTHER AFFIANTS SAYETH NAUGHT.
